NCRCP Application Process
The National Crisis Responder Credentialing Program
To submit your application, please:
| 1. | Provide the following Original documents, in this order: |
| |
| a. | Notarized NCRCP Application. |
| b. |
One or more notarized Memorandum of Confirmation Forms verifying your paid or volunteer experience. |
| c. |
Two letters of recommendation on prefessional letterhead that attest to your professional (paid or unpaid) skills, ability and amount of experience in the field. |
| d. |
One or more Intervention Evaluation Forms that encompass at least three observed individual or group crisis intervention sessions (practice or actual). These evaluation forms must be sealed in an envelope by the evaluator. |
| e. |
One Crisis Response Experience / Incidents Form for each paid or volunteer experience with an organization or team. Unaffiliated individual volunteers should follow instructions for unaffiliated volunteers. |
| f. |
For NOVA-Authorized Trainers only, the NOVA Crisis Response Trainer’s Documentation Form. |

| 2. | 2. Include a check with your application or complete the credit card information section on the NCRCP application. The fee for new applications is $150. |
| 3. |
3. Make sure all documents are signed and notarized where appropriate. |
| 4. | Do NOT fax your application as faxed applications will not be accepted |
| 5. | Do NOT staple forms |
| 6. |

7. Keep a copy of the Application packet for your records. |
| 7. |
8. Ensure that your email address is correct and legible. In the absence of an email address, please provide your fax number. You will receive confirmation of receipt of your Application via email or fax. |
Your application will be reviewed at the next upcoming Application Review date (If submitted in February, applications will be reviewed in March; if submitted in August, applications will be reviewed in September.) If approved, you should receive your mailed credential within 4-6 weeks of review. Your credential will be valid for two years from the date of that Review process. An email or fax notice will be sent reminding you of the timeline for renewal, prior to the expiration of your credentials.
